The annual subsidy inspection was conducted by the inspector using an alternate remote protocol necessary due to a state of emergency declared by the Governor of Virginia in response to the COVID-19 pandemic.

An annual subsidy inspection was initiated on May 8, 2020, at which time the vendor was contacted by telephone to initiate the inspection, and concluded on May 12, 2020 information is reviewed. The inspector reviewed 4 children's records and 2 staff records submitted by the vendor and health and safety regulations.

Information gathered during the inspection determined areas of non-compliance with applicable regulations or law, and violations were documented on the violation notice issued to the vendor.

Description: Based on record review, the Vendor failed to have required information for each enrolled child.

Licensing Inspector reviewed four (4) children records. Records for Child #1 and Child #2 did not include the work address for parents.

Plan of Correction: The Center Director will have each to complete the registration form to include the work address.
