The annual subsidy inspection was conducted by the inspector using an alternate remote protocol necessary due to a state of emergency declared by the Governor of Virginia in response to the COVID-19 pandemic.

Standard #: 22VAC40-665-520-B-11 Description: Based on record review, the Vendor failed to have a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.
EVIDENCE:
Licensing Inspector reviewed four (4) children records. Record for Child #3 did not include a written allergy care plan for the food allergy identified by the parent on the enrollment forms.Plan of Correction: Center will have parents submit allergy care plan from the child's physician.

Standard #: 22VAC40-665-530-2 Description: Based on record review, the Vendor failed to ensure each staff member has completed required Background Checks.

Licensing Inspector reviewed two (2) staff records. Record for Staff #1 did not include documentation of a completed Central Registry Background Check.Plan of Correction: Staff member will complete background check. Documentation of the findings will be maintained in the staff record. Center will submit documentation to the Licensing Inspector within 10 days.
