Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on 1/27/2021 and concluded on 1/29/2021. The person in charge was contacted by telephone to initiate the inspection. There were 34 children present with 9 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 medications and supporting documents, 3 children?s records and 3 staff records submitted by the facility to ensure documentation was complete. Violations from the previous inspection were reviewed to ensure current compliance.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-665-520-A
Description: Based on record review and interview, the center failed to ensure one out of three children's records had all the required information.

Evidence:

1. The record for child 3 did not have the name and phone number for the employer for the child's father.
2. The administrator stated they did not have the employer information for child 3's father.

Plan of Correction: The employer information will be obtained for child 3's father. In the future the administration will ensure that each child's record is complete prior to the first date of attendance.

Standard #: 22VAC40-665-580-H
Description: Based on record review and interview, the center failed to ensure staff that started prior to 3/1/2019 completed the department's health and safety update course prior to 3/1/20.

Evidence:

1. The records for three staff were reviewed.
Staff 1's start date was documented as 2/5/18. The preservice updated training was documented as completed on 7/5/20.
Staff 2's start date was documented as 8/4/17. The preservice updated training was documented as completed on 1/27/21.
Staff 1's start date was documented as 6/8/18. The preservice updated training was documented as completed on 1/27/21.
2. The administrator verified the start dates and the dates the health and safety updates were completed. The administrator stated the dates completed were the first times the staff had completed the health and safety update course.

Plan of Correction: In the future all staff will complete the department's health and safety update course within one year of taking the initial pre-service training and yearly there after.
