Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. A subsidy health and safety inspection (SHSI) was initiated on 2/4/2026 and completed on 2/4/2026. The inspection started at 11:20am and concluded at 12:00pm. There were 2 children present and 1 caregiver; the point total was 4. The inspector reviewed 2 children?s records and 3 caregiver/household members records on-site on 2/4/2026. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 2/18/2026.

Standard #: 22.1-289.040-A Description: The family day home vendor is required to have household members undergo background checks every five years.
At the time of the inspection, the most recent national criminal background check for Household Member #1 was overdue by more than two years, and the criminal background check for Household Member #2 was overdue by more than six months. Additionally, there was no sworn statement on file for either Household Member #1 or Household Member #2.Plan of Correction: Background checks will be completed within 10 calendar days, or the individuals can choose to move out of the home.
Standard #: 8VAC20-790-180-B Description: The vendor shall maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
