Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

An unannounced code compliance inspection was initiated on 4/22/2021 and concluded on 4/29/2021. The person in-charge was contacted by telephone to initiate the inspection. There were 27 children present and 3 staff. The inspector emailed the provider a list of items required to complete the inspection. A virtual tour was taken of the facility. The Inspector reviewed 3 staff records submitted by the facility to ensure documentation was complete.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 63.2(17)-1716-A-4
Description: Based on record review and interview, the center failed to ensure each staff member has been certified by a physician, physician assistant, or nurse practitioner to be free from any disability which would prevent him from caring for children under his/her supervision.

Evidence:

1. The record for staff 2 documented the start date as 9/8/20. The record did not contain documentation from a physician.
2. The director stated they did not obtain a staff health record for staff 2.

Plan of Correction: Staff 2's physician has been given the form to complete. In the future no staff will be allowed to start work without the completed staff health report.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review and interview, the center failed to obtain fingerprint-based criminal history check determination letters prior to the first day of employment.

Evidence:

1. Based on the record for staff 1 the hire date was 9/8/20. The record for staff 1 did not contain a fingerprint-based criminal history check determination letter.
2. Based on the record for staff 1 the hire date was 9/8/20. The record for staff 1 did not contain a fingerprint-based criminal history check determination letter.
3. The director stated they were unable to get an appointment at the normal location at the time and were unaware of alternate processes and locations.

Plan of Correction: In the future if there are difficulties in completing requirements the director will contact the licensing inspector for directions. No staff will be allowed to start work without the fingerprint-based criminal history check determination letter in the future. Both staff have appointments to get fingerprints on 4/29/21.
