Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documentation review, interviews and a virtual tour of the program.

A code compliance inspection was initiated on 08/18/2021 and concluded on 08/18/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 35 children present with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and medication. A total of 3 children?s records and 4 staff records were reviewed.
Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on staff record review, the center did not ensure one of four staff records had documentation of central registry results within 30 days of employment.
Evidence:
1. The record of staff #4 (start date: 10/18/20) had documentation of central registry results dated 2/09/21.

Plan of Correction: All staff will obtain Registry results prior to employment. We have implemented an employee profile for each staff with all necessary requirements, orientation training, and paperwork training. This will help us to see what has or has not been completed. Ms. Ella, Director will be responsible for making sure all paperwork and training has been completed prior to working and will monitor to make sure this does not reoccur. Date corrected 8/18/21

Description: Based on observation and staff interview on 08/18/2021, the center did not ensure at least one staff member was regularly present in each grouping of children.
Evidence: 1. There was one volunteer with 18 children in the school age classroom. 2. The center representative stated that the volunteer was to be included in the class ratio, however the volunteer was not considered a staff and had only completed a national criminal history search for volunteers.

Plan of Correction: Effective 8/18/21 all volunteers will be trained and required all necessary paperwork as we do our staff. We have implemented an employee profile for each staff with all necessary requirements, orientation training, and paperwork training. Profile example attached. The individual volunteering that day has completed all paperwork required. Ms. Ella, Director, will be monitoring and responsible for this to not occur again. Date of correction 8/19/21

Description: Based on staff record review, the center did not ensure one of four staff records had documentation of fingerprint based national criminal history search results prior to employment.
Evidence:
1. The record of staff #4 (start date: 10/18/20) had documentation of fingerprint based national criminal history search results dated 12/18/20.

Plan of Correction: Effective 8/18/21 All staff will receive fingerprinting prior to employment. This staff was not able to get fingerprinted in Oct. 2020 due to the office was shut down for COVID-19 and the office reopened in December of 2020. We have implemented an employee profile for each staff with all necessary requirements, orientation training, and paperwork training. This will help us to see what has or has not been completed. Ms. Ella, Director, will be monitoring and responsible for this to be implemented prior to work and not to occur again. Date corrected 8/18/21
