Technical Assistance:
Technical assistance today included record keeping, program and the physical environment.

An unannounced monitoring inspection was conducted this afternoon from 1:20 - 3:30 pm. There were 10 children present with the provider and two assistants for 30 points. I observed children at nap; and crawling, cuddling and playing with toys. The new arrangement of cribs opens the children's area for more movement activities; and the two new toddler sized toy bins can really promote independence. Description: A review of records today found that two children in care did not have documentation of a physical examination within 30 days of beginning the program. Evidence: Child # 1, who began care 3/14; and Child # 2, who began care 10/14/14, both did not have physical examinations available today.

Plan of Correction: I will remind the parents today.

Description: A review of the records of assistants today found some required information was not available. Evidence: Assistant #2 had only one reference check (2 are required.)

Plan of Correction: I will check on another reference.

Standard #: 22VAC40-111-60-B
Description: A review of children's records today found that some information was not available. Evidence: Of 8 records reviewed today, Child # 4 only had one emergency contact listed. Child # 3 had no documentation that the contact information was reviewed and updated by parents annually as required (last updated 8/13.)

Plan of Correction: I will talk to the parents today.

Description: A review of children's records today found that not all children had proof of identity information as required. Ev idence: A review of 8 children's files found that there was no documentation of proof of identity for Child # 2, who began care 10/14/14, over a month ago.

Plan of Correction: I have asked the parent. I will remind them.

Standard #: 22VAC40-111-80-D
Description: A review of children's files today found that the provider did not notify local law enforcement as required when a parent did not provide proof of identification within 7 days of beginning care. Evidence: The provider did not notify authorities after 7 days when the parent did not bring in the required proof of identification.

Plan of Correction: I will tell the parent that this is the requirement.

Standard #: 22VAC40-111-170-A
Description: The provider did not obtain from one assistant a report of a tuberculosis screening as required before coming into contact with children. Evidence: Assistant #2, who began work with children 10/14, did not have documentation of a TB screening available today.

Plan of Correction: She will go tonight.

Standard #: 22VAC40-111-200-A
Description: A review of staff records found that there was no documentation of an orientation to the position for Assistant #2.

Plan of Correction: I will get the form and we will go over it.
