The licensing inspector conducted an unannounced renewal inspection from 9:00am - 11:15am. There were five children in care for a total of nine points with one caregiver. The children were observed at free play, watching cartoons and playing with play dough. One caregiver, three household members and two children's records were reviewed. Violations from the previous inspection were also reviewed and one was repeated. Areas of non-compliance are identified on the violation notice. If you have any questions, please contact your licensing inspector at (276) 676-5634. Thank you for your cooperation.
Standard #: 22VAC40-191-40-D-4-a Description: Based on record reviews, the family day home failed to obtain a background check on an 18 year old residing in the home. Evidence: 1. The inspector reviewed three household member's records. One of three records reviewed (household member record #2) did not contain a criminal history record check and household member #2's date of birth is 10/02/95. Household member record #2 did contain a search of the central registry and a sworn disclosure statement. Plan of Correction: The provider will request a criminal history record check on the household member. The provider will review the household member's records quarterly to ensure all information is complete and up to date.
