An unannounced renewal inspection was conducted beginning at 1:30 p.m. and ending at 3:30 p.m. with the provider. There were four children in care ranging in age from ten months to four years old, totaling ten points. Children were observed having free play and watching an age appropriate television program. After free play, children settled for nap time. An infant was observed having a snack and sleeping. There was a school delay this morning due to weather and the day care schedule was behind. Three provider records, three household records and three children's records were reviewed. Emergency drills were reviewed and up to date. The Emergency Preparedness Plan (EPP) was reviewed and up to date. The weekly menu was reviewed and posted. Annual training was reviewed. The provider has completed twelve hours of training and has until 1/31/2015 to complete four additional hours of training for the license year. The provider does not administer medication and is not Medication Administration Training (MAT) certified. Standard #: 22VAC40-111-180-A
Description: Based on review of provider records, not all TB tests were updated every two years as required. Evidence: The most recent TB test for Provider A and Provider B was dated on 11/20/2012.

Plan of Correction: The TB tests will be updated as soon as possible.

Standard #: 22VAC40-111-320-B
Description: Based on observation, not all electrical outlets accessible to children under the age of five had protective covers as required. Evidence: Three of six outlets, located near the napping area of a three and four year old in care, were not covered.

Plan of Correction: The outlets were covered during the inspection.

Standard #: 22VAC40-111-520-A
Description: Based on observation and interview with the provider, a crib was not provided for a child under the age of 12 months. Evidence: The provider was using the only crib for a nap space for a two and a half year old and did not have a crib for an infant, age 10 months.

Plan of Correction: The crib will be used for the infant in care. The beds and nap mats will be used for the older children in care.
