An unannounced monitoring inspection was conducted on 3/11/15 from 11:53am - 1:00pm. At the time of entrance there were six children ages one to four years old (15 points) in care with two caregivers. A tour of the home was conducted and children were observed sleeping during rest period. Records were reviewed for six children and three provider/assistant provider/household members. Emergency plan and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).
Standard #: 22VAC40-111-790 Description: Based on observation, the provider did not ensure that 911 or a local dial number for police, fire and emergency medical responders, the phone number for the person responsible for emergency backup care, and the phone number for the regional poison control center were posted in a visible area close to the telephone. Evidence: 1. The emergency phone number for police, fire and emergency medical responders, the phone number for the person responsible for emergency backup care, and the phone number for the regional poison control center were not posted in a visible area close to the telephone. 2. The provider confirmed that the required phone numbers were not posted. Plan of Correction: The required telephone numbers were posted during the inspection.
Standard #: 22VAC40-111-350-E Description: Based on observation, the provider did not ensure that hot water at taps available to children shall be maintained within in a range of 105 degrees Fahrenheit to 120 degrees Fahrenheit. Evidence: The water temperature at the faucet in the bathroom used by the children was 125.2 degrees Fahrenheit. Plan of Correction: The temperature for the hot water was turned down during the inspection and will be monitored to ensure it stays within the required temperature range.
