54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted on June 3, 2015. The provider and her two assistants were present with 3 children. (10 points) One of the children, an infant, was being held and fed a bottle. He later was placed in his crib. The second infant was in her crib, and the preschool age child was putting together a puzzle. He later played with blocks. Consultation was provided on the prohibition of the use of infant walkers. Inspection began: 10:30 Inspection ended: 11:50
Standard #: 22VAC40-111-180-A Description: Based on a review of 3 caregiver records, the provider had not obtained an updated TB test/screening on one of the caregivers. Evidence: The TB test/screening form for Caregiver # 2 was not completed with the results and date. Plan of Correction: The TB test/screening will be done June 9, 2015.
