Comments:
An unannounced monitoring inspection was conducted on 6/4/15 between 3:35 and 5:30. There was a total of seven children present with two providers for a total of 11 points. Two household children were also present after school. The inspector spoke with the provider and assistant and with the children who were present. The children were observed playing with toys, games and eating snack. The children were seen washing their hands prior to snack. Snack today was graham crackers, bananas and applesauce. Last evacuation drill was on 5/5/15 and a shelter-in-place drill was conducted on 1/13/15. Records for the seven new children were reviewed. The record for the new assistant was also reviewed. All required documents were posted including license, inspection information, menu, evacuation map.

Violations:
Standard #: 22VAC40-111-960-K
Description: The provider failed to ensure that infants being fed baby food are fed from a dish and not from the baby food jar. Evidence: When the inspector first arrived at the home, the assistant and two infants were at the home. The provider was picking up children at school. The assistant was preparing to feed the infants and had two jars of baby food open. The assistant did not have a dish to put the food into before feeding the children. The provider acknowledged that the assistant had fed the infants out of the baby food jar.

Plan of Correction: Assistant was informed that infant's food shall be feed in a bowl at all times.

Standard #: 22VAC40-111-60-B
Description: The provider failed to obtain the full addresses for the emergency contacts for five new children (siblings). Evidence: The inspector reviewed the records for seven new children. Five of the children (#1 through #5) are siblings. The records showed the names and phone numbers of two people listed for the emergency contacts. None of the records gave addresses for the two emergency contacts listed for all of the children. The provider contacted the parent and requested the information. The information was added to the children's records.

Plan of Correction: Corrected during inspection.
