Technical Assistance:
Emergency MAT training information was given to the provider. Requirements for substitute provider records were reviewed. An exit interview was conducted at the end of the inspection.

Two licensing inspectors conducted an unannounced licensing study from approximately 9:45 am until 11:15 am. There were ten children in care for a total of twenty-two points with the provider and assistant providing supervision at all times. Children were observed playing inside and outside, washing hands, having a snack, watching a video, singing songs, and playing. Three children's files were reviewed and three staff files were reviewed. The violation from the last inspection had been corrected. Violations from today's inspection are identified on the violation notice. Please call your inspector at (276) 676-5492 for any information.

Description: Based on observation, the provider did not follow the most recent, age-appropriate requirements of a recognized authority. Evidence: 1. The inspector observed a snack of goldfish and water being served for snack. Snacks must have two food groups.

Plan of Correction: The provider and assistant will start serving snacks with two food groups.

Description: Based on observation, the provider did not have snacks listed on the menu. Evidence: 1. The inspector reviewed the menu and it did not have snacks listed.

Plan of Correction: The provider will start to list snacks on the weekly menu.
