Discussed with the provider that a maximum of 16 points are allowed per caregiver when determining is another caregiver is needed. The provider's own children and children that reside in the home are not included in the capacity but are included in the total points.
An unannounced monitoring inspection was conducted today from 11:00am-12:10pm. There were 7 children(4 resident children)(18 points) directly supervised by the provider. The home, 1 staff record, 4 children?s records, evacuation drills and emergency supplies were inspected. There were no medications on site today. Children were observed playing freely and eating lunch. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-830-D Description: Based on provider statement, records of emergency evacuation drills were not maintained for one year. Evidence: An emergency evacuation drill was observed documented on June 10, 2015, however previous month's drills were not documented. The provider stated that the drills had been conducted but she threw away the old sheet once it was finished. Plan of Correction: I will keep all records of emergency drills.
Standard #: 22VAC40-111-960-E Description: Based on direct observation, bottles were not labeled with the child's full name and the date. Evidence: Three bottles were observed in the home and were not labeled with the child's name or the date. Plan of Correction: I will label with tape.
Standard #: 22VAC40-111-100-A Description: Based on record review, the provider did not obtain documentation of children's physical examinations prior to or within 30 days after the first day of attendance. Evidence: Documentation of the physical examination results was not observed for Child 3 and Child 4, who both enrolled November 18, 2014. Plan of Correction: I will ask the parent today.
Standard #: 22VAC40-111-50-A Description: Based on record review, the provider did not keep a written record of children in attendance each day. Evidence: Documentation of the attendance record was last completed on June 22, 2015. Plan of Correction: I will complete the attendance.
Standard #: 22VAC40-111-60-B Description: Based on record review, children's records were incomplete. Evidence: 1. A second emergency contact person was not observed documented for Child 1 and Child 2. 2. The parent's work address and phone number, the doctor's street address, the emergency contacts' street address, verification of the proof of age and identity and the authorization for emergency medical care were not observed documented for Child 3 and Child 4. Plan of Correction: I will ask the parents today.
Standard #: 22VAC40-111-90-A Description: Based on record review, the provider did not obtain documentation of children's immunization records prior to attendance. Evidence: Documentation of immunization records were not observed for Child 3 and Child 4, who both enrolled November 18, 2014. Plan of Correction: I will ask the parent today.
Standard #: 22VAC40-111-320-B Description: Based on direct observation, electrical outlets did not have protective covers. Evidence: Three outlets were observed without protective covers in the living room area of the home. Plan of Correction: I will cover.
Standard #: 22VAC40-111-570-A Description: Based on direct observation, the provider did not ensure that the maximum of 16 points per caregiver was not exceeded to determine if an additional caregiver was needed. Evidence: During the inspection, the provider was caring for a total of 7 children(4 resident children, 2 children enrolled in care, & 1 child whom she was baby-sitting for the day), which totaled 18 points, thus exceeding the maximum of 16 points that is allowed per caregiver. Plan of Correction: I am baby-sitting one baby only for today. I will monitor my points.
Standard #: 22VAC40-191-40-D-4-a Description: Based on record review, the provider did not obtain a sworn statement or affirmation upon a person residing in the home or a search of central registry and criminal history background check within 30 days of a person residing in the home. Evidence: The sworn statement of affirmation, the search of central registry, and the criminal history record background checks were not observed for Household Member B. Plan of Correction: I have been trying to get her to complete the forms but she has not completed them yet.
