54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted with the provider and assistant beginning at 9:45 a.m. and ending at 12:00 p.m. There were twelve children in care ranging in age from eight months to eleven years old for a total of twenty-three points. Children were observed having free play with blocks. After free play, children had outside play time in the backyard. The daycare area was observed. Four children's records, three provider records and a household member record were reviewed. The emergency drills were reviewed and up to date. The attendance record was reviewed and complete. Standard #: 22VAC40-111-60-B Description: Based on review of documentation, not all children's records were complete. Evidence: Child A, enrolled 2/1/2010, did not have a second emergency contact. Plan of Correction: We will get the information from the parent at pick up today.
Standard #: 22VAC40-111-690-D Description: Based on observation, the diapering pad was not nonabsorbent. Evidence: The diapering pad had a tear in the middle center area exposing the absorbent filling inside. Plan of Correction: The diapering pad will be replaced as soon as possible.
