- Technical Assistance:
Discussed the following with the Provider: - a letter the Provider received regarding the Department repealing and replacing the General Procedures - medication authorizations for children in care - changes to the outside play area that the Provider is looking to make in the near future - background checks for a volunteer with the FDH
An unannounced monitoring inspection was initiated at 1:50pm. This was in response to a concern that the Provider made to the Licensing Inspector regarding an allegation of an incident at the FDH. There were no violations as a result from this inquiry. Today, there were 4 children in care, totaling 6 points, under the supervision of the Provider. During the inspection, children were observed coming in from playing outside, enjoying free play, playing the piano, working on their summer journal, and preparing to make brownies. Earlier today, the children made play-doh with the Provider. For lunch, the children were served tuna on whole wheat bread, broccoli, oranges and milk. This past May, the Provider completed a CDA class. She will now complete observations and take the exam to become CDA accredited. She is currently certified in CPR, First Aid and MAT (to administer medications). Should you have any questions, please contact me.
Standard #: 22VAC40-111-100-A Description: Based on file review, 1 out of 4 children's files does not have the results of a physical within 30 days of the child receiving care at the FDH. Evidence 1. Child #1 began care on 6/8/15. There is no physical on file. Plan of Correction: The Provider will talk to the parent today about bringing this to the FDH.
Standard #: 22VAC40-111-70-A Description: Based on file review, 1 out of 4 children's files does not have verification that parents received all required information before the child's first day of attendance Evidence 1. Child #1 began care on 6/8/15. The parent has not signed the "Information for Parents" or Medication Policy form. Plan of Correction: The Provider is aware of this and will make sure to have this corrected today.
