54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted from 12:05 pm - 1:20 pm. There were 8 children in care with the provider and assistant ages 10 months, 15 months, 16 months, 2 years, 2 years, 2 years, 3 years, and 4 years for a total of 21 points. The children were observed napping and having free play. Lunch served today was chicken patty, peas, french fries, and milk. Eight children's records were reviewed. The provider and assistant records were reviewed and in compliance. The most recent fire drill was conducted 7/2/2015. The provider is current in CPR and First Aid. The provider does not administer medications to children in enrolled at the family day home. MAT (Medication Administration Training) is required to administer medication to children enrolled in the family day home. Standard #: 22VAC40-111-100-A Description: Based on review of 8 children's records it was determined that not all records were complete. Evidence: Child B with a start date of 6/9/2015 and child C with a start date of 5/28/2015 were missing documentation of a physical before 30 days of enrollment. Plan of Correction: The provider will request copies of the most recent physicals from the parents of child B and child C.
Standard #: 22VAC40-111-60-B Description: Based on review of 8 children's records it was determined that not all records were complete. Evidence: Child A with a start date of 9/21/2014 was missing one of the two required emergency contacts. The emergency contact documented for Child A was missing the address and phone number. Plan of Correction: The provider will request the missing information from the parent of child A.
Standard #: 22VAC40-111-90-A Description: Based on review of 8 children's records it was determined that not all records were complete. Evidence: Child B with a start date of 6/9/2015 did not have documentation of immunizations before the first day of enrollment. Plan of Correction: The provider will request documentation of the most recent immunizations from the parents of child B.
