Technical Assistance:
Effective August 1, 2015, VDSS will increase the fee for Child Protective Services Central Registry Search request based on actions taken by the 2015 General Assembly in the State Agency Savings Plan. The fee will increase from $7.00 to $10.00 beginning August 1, 2015.

Comments:
The licensing inspector conducted an unannounced monitoring inspection on Monday, July 27, 2015 from 10:05am to 11:10am. The children were observed doing music and movement to a Baby Dance dvd and playing musical chairs. They were later observed calming down to soothing music. Today, there were six children, totaling 13 points, present with one caregiver and one volunteer. Lunch consisted of grilled cheese, mixed fruit, carrots and milk. Two children?s records and one caregivers? record were reviewed during the inspection. Last emergency drill: 7/11/2015 Last shelter-in-place drill: 6/1/2015 Emergency Plan Reviewed:9/2/2014 The violations from the previous inspections in were checked for corrections. Description: Based on a review of children's records on 7/27/2015, the provider did not ensure that each child's record contained documentation of two emergency contacts. EVIDENCE: The record of Child #2 contained information of one emergency contact. Each child's record shall contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached.

Plan of Correction: Will have parent complete missing information today, 7/27/2015.
