Comments:
Today an unannounced monitoring visit was conducted beginning at 11am. There were 8 children present (17 points) with the Provider and assistant caregiver. Children were reading, coloring and preparing for lunch. There are no children enrolled with medications at this time. This inspection, including the exit interview, ended at 12:10pm. Standard #: 22VAC40-111-800-B
Description: Based on record review the emergency plan has no documentation that it was reviewed and updated annually. Evidence: There is no documentation that the emergency preparedness plan has been updated anytime between July, 2014 and July, 2015.

Plan of Correction: I will review and update the plan today and annually.

Standard #: 22VAC40-111-60-A
Description: Based on record review, two children's records (#3 and 8) do not have documentation of up-to-date records. Evidence: Records for children #3 and 8 do not have documentation that their files have been reviewed and updated within the last year by the parents. Child #3 last had an update 7/24/14 and child #8 last had an update 3/14/14.

Plan of Correction: I will have the parents review and update these records.

Standard #: 22VAC40-111-60-B
Description: Based on record review, one child's record (#9) is missing complete required documentation. Evidence: Child #9 is missing documentation of a second emergency contact with a complete address.

Plan of Correction: I will ask the parents to give me a second emergency contact.

Standard #: 22VAC40-111-210-A
Description: Based on record review, there is no documentation that 16 annual training hours were attended by caregiver #1. Evidence: Only 5 of 16 annual training hours could be located.

Plan of Correction: I will continue to collect them fr om the last year. I know I attended 16 hours. I will be sure to save all documentation of training hours for this year as well.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review, one adult record (#1) does not have a background check updated within 3 years of the most recent results. Evidence: The most recent results on file for a search of the central registry results is dated 2/2012.

Plan of Correction: We will submit background checks every three years and submit this one immediately.
