Comments:
An unannounced monitoring/renewal inspection was conducted today from 11:50am-12:30am. There were 11 children (32 points) directly supervised by 3 staff. The home, 2 staff records, 5 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and eating snack. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Standard #: 22VAC40-111-60-B
Description: Based on review, the provider failed to ensure that all children's records are maintained with the required information. Evidence: Reviewed 5 children's records and found that CH3 and CH5 did not have documentation of current annual updated/review by parents. CH3's last update was on 6/23/14 and CH5's last update was on 7/16/14.

Plan of Correction: Got it reviewed and signed.

Standard #: 22VAC40-111-90-C
Description: Based on review, the provider failed to ensure that immunization records for children under 2 years of age are updated every 6 months. Evidence: Reviewed 5 children's records and found that CH5's last documented immunization was on 8/18/14

Plan of Correction: Parent brought the new updated health form.

Standard #: 22VAC40-111-180-A
Description: Based on review, the provider failed to ensure that all caregiver's TB screenings are completed every 2 years from the date of the first screening or more frequently as recommended by a physician. Evidence: Reviewed one staff record and found that the most recent TB screening was on 6/13/13.

Plan of Correction: Made the appointment.
