Technical Assistance:
Discussed with the provider that all original background checks for staff and household members that are returned to the provider from the appropriate agency should be maintained at the home and should not be released.

Comments:
An unannounced monitoring inspection was conducted today from10:00am-11:30am. There were 8 children(22 points) directly supervised by 3 staff. The home, 3 staff records, 4 children?s records, 1 household member?s record, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing outside. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-111-120-A
Description: Based on record review, staff records were incomplete. Evidence: Documentation of at least two references was not observed for Staff B and Staff C.

Plan of Correction: I will have them next week.

Standard #: 22VAC40-111-60-B
Description: Based on record review, two of four children's records were incomplete. Evidence: 1. The street address for the emergency contacts listed for Child 1 was not observed documented. 2. An annual update of children's information was last conducted on June 16, 2014 for Child 2.

Plan of Correction: Today I will ask the parents.

Standard #: 22VAC40-191-60-C-1
Description: Based on record review and provider statement, the provider did not have an original criminal history record report within 30 days of employment for caregivers. Evidence: Documentation of an original criminal history record report was not observed for Caregiver B (hired March 16, 2015) and Caregiver C (hired June 20, 2015). Photocopies of the completed documents were observed. The provider stated that the original documents had been retrieved by another local agency.

Plan of Correction: I will ask the county to return them.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and provider statement, the provider did not have an original CPS central registry finding within 30 days of employment for caregivers. Evidence: Documentation of an original CPS central registry finding was not observed for Caregiver B (hired March 16, 2015) and Caregiver C (hired June 20, 2015). Photocopies of the completed documents were observed. The provider stated that the original documents had been retrieved by another local agency.

Plan of Correction: I will ask the county to return them.

Standard #: 22VAC40-80-120-E-2
Description: Based on direct observation, the findings of the most recent inspection was not posted in the home. Evidence: Documentation of the most recent inspection (conducted January 23, 2015) was not observed posted in the home.

Plan of Correction: I will post the new violation notice once received.
