- Technical Assistance:
Discussed the following with the Provider: -the possibility of increasing the FDH capacity and what steps to take - timeframes for background checks to be completed. The LI printed a sworn disclosure statement form for the Provider to use.
An unannounced monitoring inspection was initiated today at 2:30pm. There were 4 children (plus the Provider's child), totaling 8 points, under the supervision of the Provider. The inspector arrived to the FDH at 12pm, but the Provider had taken the children on a special field trip to McDonald's for ice cream. During the inspection, children were observed taking naps and interacting with the Provider in a positive and constructive manner. For lunch, the children were served chicken nuggets, broccoli, mashed potatoes, carrots and milk. The Provider is certified in MAT (to administer medications), CPR and First Aid. Standard #: 22VAC40-111-920 Description: Based on observation, the posted menu is not for the current one-week period. Evidence 1. There was a printed monthly menu, but it was for the month of May 2015. A dry erase board had the menu of what was served to the children today. Plan of Correction: The Provider will post a weekly menu and make sure that it is current.
Standard #: 22VAC40-191-40-D-1-c Description: Based on file review, 3 out of 5 caregiver/household member files do not have background checks completed within the past three years. Evidence 1. Caregiver/household members #1, #2 and #3 have sworn disclosure statements (SDS), criminal history record checks (CHR) and a search of the central registry (SCR) which were last completed in June 2012. Plan of Correction: The Provider sent the CHR and SCR out at the beginning of August. The SDS will be completed this week.
