Technical Assistance:
**At the time of the inspection the provider stated that they had recently had a flood that destroyed their children's records and that they are currently working to have parents replace the children's records and that they will have them during the next inspection visit.**

Comments:
An unannounced monitoring/renewal inspection was conducted today from 10:45am-11:30am. The home, 2 staff records, 3 household member?s records, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Description: Based on observation, the facility failed to ensure that the emergency operated radio was working. Evidence: Observed that the emergency operated radio did not have working batteries.

Description: Based on observation and staff interviews, the providers failed to ensure that they were keeping a written record of attendance each day. Evidence: Observed and identified through staff comments that there was no attendance sheet available for review.

Description: Based on review and staff interviews, the facility failed to ensure that background checks were obtained for all household members every 3 years. Evidence: Reviewed documentation from previous inspection and identified through staff interviews, that the background checks for Household Member (HM)s were not completed: HM1: expired Criminal History and CPS check HM2: expired Criminal History, CPS check and Sworn Disclosure.
