Discussed this years hours, the provider is open from 3pm to 6pm at this time.
The provider is present with 9 children totalling 10 points. The children have eaten snack, today's snack was goldfish and water. The children are doing homework, playing games and playing with cars and other toys. The provider has lots of supplies and space for the children to explore. Lots of interaction between children and the provider observed during the visit. This visit was started at 3:30 and ended at 4:25. Standard #: 22VAC40-111-100-A Description: The provider failed to get a physical form from one child out of 4 children's files reviewed. Evidence: Based on the review of 4 children's files one child, child A is missing a physical form. Plan of Correction: I have asked the parent for information and told her to get a copy of the school physical.

Evidence: Based on the review of the providers records the background checks were last conducted in January of 2012.
