Technical Assistance:
Immediately correct all violations.

Comments:
An unannounced monitoring inspection was conducted today with the provider and her assistant and 10 children for a total of 22 points. Most of the children were sleeping at the beginning of the inspection but woke up mid-way through the inspection. For lunch, the children were provided Salisbury steak, noodles, bread, green beans, strawberries/grapes and milk. Arrival time: 2:50 pm Departure time: 3:40 pm

Violations:
Standard #: 22VAC40-111-830-A
Description: Based on a review of the escape drill log and an interview with the provider, it was determined that the provider did not do practiced monthly drills. Evidence: There was no monthly escape drill for August 2015.

Plan of Correction: We will remember to do monthly escape drills.

Standard #: 22VAC40-111-920
Description: Based on observation and an interview with the provider, it was determined that the menu for the current one week period was not posted or provided to the parents. Evidence: There was no menu posted or provided to parents for the one week.

Plan of Correction: We will post the menu for the one week period.

Standard #: 22VAC40-111-100-A
Description: Based on a review of six children's files and an interview with the provider, it was determined that one child did not have a physical record on file within 30 days of enrollment. Evidence: Child A ( enrolled on 1/2/12 )does not have a physical on file

Plan of Correction: We will get child A's physical.

Standard #: 22VAC40-111-90-A
Description: Based on a review of six children's files and an interview with the provider, it was determined that one child did not have an immunization record on file prior to enrollment. Evidence: Child A ( enrolled on 1/2/12 )does not have an immunization record on file.

Plan of Correction: We will make sure to obtain the child's immunization record.

Standard #: 22VAC40-111-180-A
Description: Based on a review of files and an interview with the provider, it was determined that person A does not have a tuberculosis test/screening every two years. Evidence: Person A's tuberculosis test was dated 2/2/13.

Plan of Correction: We will get Person A to do an up-to-date test/screening.

Standard #: 22VAC40-111-320-A
Description: Based on observation, it was determined that a small appliance was plugged into an outlet while not in use. Evidence: The Licensing Inspector observed a toaster plugged into an outlet, the toaster was not in use.

Plan of Correction: The toaster was unplugged during the inspection. We will remember to unplug when not in use.
