An unannounced complaint investigation regarding the care and supervision of children was conducted today. Staff interviews and programming observations were completed today. After careful review of all available information, this complaint was deemed not valid. However, violations not associated with the complaint were cited today. The provider and one assistant caregiver were caring for 8 children, totaling 20 points. The children were observed assembling puzzles, reading books, playing with table top toys, and preparing for lunch. The children were served cheese pizza, applesauce, and milk for lunch today. Description: Based on observations, it was determined that one piece of equipment has not been maintained in a clean and operable condition. Evidence: The diaper changing pad has torn and ripped in various places.

Plan of Correction: I bought a new changing pad earlier this summer. The new changing pad was put in place during the inspection.

Description: Based on observations, it was determined that one electrical outlet was not covered. Evidence: Upon arrival, one electrical outlet on the bottom half of the wall was not covered.

Plan of Correction: The outlet was covered during the inspection.

Complaint related: No
Description: Based on observations, it was determined that one infant was placed in a crib without a soft item. Evidence: The Inspector observed one infant (age 4 months) being placed in his crib for a nap. The caregiver placed the child in the crib with his bib still in place around his neck.

Plan of Correction: The bib was removed immediately during the inspection.

Complaint related: No
Description: Based on observations, it was determined that the provider did not conduct activity time in an age-appropriate manner for one child. Evidence: One child (age 3 years and 8 months) was seated in a high chair during book/puzzle/table top activity time. This child was too big for the high chair, and was not permitted to sit at the table alongside the three other preschool children (age 4 years.) It was reported to the Inspector that this child was seated in the high chair because he doesn't stay seated during table activity time.

Plan of Correction: The child was moved from the high chair to the table during the inspection. He likes to get up and move around when the children are supposed to be sitting at the table. In the future, we will have the child sit at the table with his friends and make sure he is fully engaged in the activities so that he stays seated.
