22VAC40-111-(11)-800 Discussed with the provider what is required for the annual review of the emergency plan for the family day home.
An unannounced monitoring inspection was conducted on 9/11/15from 10:20am - 12:00pm. At the time of entrance there were 11 children ages two years old to five years old (22 points) in care with two caregivers. A tour of the home was conducted and children were observed playing inside and outside with toys, and doing arts and crafts projects. Records were reviewed for five children and six caregivers/household members. There is no medication at the family day home. Emergency plan and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).
Standard #: 22VAC40-111-60-B Description: Based on a review of five children's records, it was determined that the provider did not ensure that they maintain and keep at the family day home a complete record for each child enrolled that contains all required information. Evidence: 1. The record for child #2, present during the inspection, did not contain documentation of the name and policy number of the child's medical insurance. 2. The record for child #3, present during the inspection, did not contain documentation of parent notification for the policies for administration of medication and whether or not there is liability insurance for the family day home. 3. The record for child #4, present during the inspection, did not contain documentation of parent notification for whether or not there is liability insurance for the family day home, and did not have any indication as to whether or not the children previously attended childcare. 4. Caregiver #1 confirmed the records for child #2, child #3, and child #4 were not complete. Plan of Correction: The missing information for child #2, child #3, and child #4 will be requested from their parents. All records will be reviewed upon enrollment to ensure they are completed.
Standard #: 22VAC40-111-90-A Description: Based on a review of five children's records, it was determined that the provider did not ensure that there was documentation that each child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations before the child can attend the family day home. Evidence: 1. The record for child #4 (date of enrollment 9/10/15) and present during the inspection did not contain documentation of a record of immunization. 2. Caregiver #1 confirmed that the immunization for child #1 had not been received. Plan of Correction: The parents of child #4 will be asked to provide a copy of the immunization record. All new children enrolled in the family day home will provide an immunization record before they start.
