54.1-3408 Provider must be MAT certified to administer prescription medication.

Comments:
An unannounced renewal inspection was conducted on September 11, 2015. There were five children present with the provider totaling seventeen points. The children were observed playing freely and watching children's programming. There are no medications being administered at this time. There is a total enrollment of six children. There is no transportation being provided by the provider. One child's record was reviewed and four household members records were reviewed during the inspection.

Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on observation and inspection of the family day home, the provider did not ensure that the first aid kit contained all required supplies. Evidence: During the inspection conducted on September 11, 2015, the first aid kit did not include adhesive bandages in assorted sizes.

Plan of Correction: Bandages will be purchased today.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider did not ensure that before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized. Evidence: During the inspection conducted on September 11, 2015, there was no documentation of immunizations in the record for Child #5. The child was present on the date of the inspection.

Plan of Correction: I have already spoken with the parent about obtaining the immunizations. The parent will give me a copy and that will be placed in the child's record.

Standard #: 22VAC40-111-570-A
Description: Based on observation and inspection of the family day home, the provider did not ensure that a caregiver does not exceed 16 points. Evidence: During the inspection conducted on September 11, 2015, the licensing inspector observed the provider working alone with five children totaling 17 points.

Plan of Correction: An assistant will be provided when points are exceeded.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review, in two of four household records reviewed, the provider did not ensure that anyone living in the family day home who is required to have background checks before three years since the dates that the last background checks were conducted. Evidence: 1. During the inspection conducted on September 11, 2015, the criminal history record checks for household member #2 and household member #3 had expired and were dated 8/20/12. 2. The search of central registry for household member #2 had expired and was dated 9/6/12.

Plan of Correction: The paperwork will be filled out and mailed tomorrow.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on review, the provider did not ensure that a child protective services central registry check was obtained within 30 days of a person in the home becoming 14 years old. Evidence: During the inspection conducted on September 11, 2015, household member #4 turned 14 years old on February 4, 2015 and there was no documentation of a child protective services central registry check.

Plan of Correction: The paperwork will be completed and mailed on Saturday.
