Comments:
An unannounced renewal study was conducted from 10:00 a.m-12:30 p.m. with the provider. Six children, ages 2 years, 3 years, 3 years, 3 years, 3 years, and 4 years, were in the care of the provider for a total of 12 points. Children were observed playing with toy cars, building with Mega Blocks, watching a cartoon, eating a lunch, and having outdoor play. Lunch served today was ham and cheese sandwich, corn, applesauce, chips, and milk. The last fire drill was 9/8/15. The first aid kit is complete. The weatherband radio and flashlight are present and accessible. The provider is current in first aid and CPR. Two household members' records were reviewed and in compliance. Standard #: 22VAC40-111-100-A
Description: Based on review of seven children's records, the provider did not obtain documentation of a physical examination prior to or within 30 days after a child's first day of attendance. Evidence: Child C's (enrolled 9/16/13) record did not have documentation of a physical examination.

Plan of Correction: The provider will notify the parents and request a copy of the physical.

Standard #: 22VAC40-111-60-B
Description: Based on review of seven children's records, the provider did not maintain complete children's records. Evidence: Child A's (enrolled 3/10/14) record did not have a parent's employer address and employer phone number. Child B's record did not include date of first attendance.

Plan of Correction: The provider will notify the parents and acquire the missing information.

Standard #: 22VAC40-111-280
Description: Based on observation, the provider did not keep potentially poisonous substances such as cleaning agents inaccessible to children. Evidence: The unlocked utility room near the basement daycare area had an unlocked gray storage cabinet that contained items such as bug spray, laminate floor cleaner, bathroom cleaning spray.

Plan of Correction: The provider will put a lock on the gray storage cabinet.

Standard #: 22VAC40-111-320-B
Description: Based on observation, the provider did not cover all unused electrical outlets accessible to children under five years of age with child-resistant protective covers. Evidence: There were five uncovered outlets in a room of the basement daycare area.

Plan of Correction: The provider will cover these outlets.
