An unannounced monitoring inspection was conducted from 9:45 a.m.-11:45 a.m. with the provider. Four children, ages 2 years, 2 years, 2 years, and 4 years, were in the care of the provider for a total of 8 points. Children were observed singing and dancing, riding tricycles, having morning snack, painting pumpkins, and practicing a fire drill. Morning snack today was apple slices and Cheerios. The last fire drill was 9/3/15. The last shelter-in-place drill was 8/5/15. The weatherband radio and flashlight were present and accessible. The provider is current in first aid, CPR, and medication administration training (MAT). Three household members' records were reviewed and in compliance. Four children's records were reviewed. Standard #: 22VAC40-111-780-B Description: Based on review of four children's records, the provider did not have an annual parent review of a child's emergency contact information. Evidence: Child A's (enrolled 12/19/11) record was last reviewed by the parent on 3/11/13. Plan of Correction: The provider will have the parent review this and sign at pick-up.
Standard #: 22VAC40-111-60-B Description: Based on review of four children's records, the provider did not maintain complete children's records. Evidence: Child B's (enrolled 12/19/11) record did not include the street address of a parent's place of employment. Plan of Correction: The provider will have the parent provide this information at pick-up.
