An unannounced monitoring inspection was completed on 9/30/15. There were five children present with the substitute assistant supervising at arrival of the inspector. The point total was 10. The provider arrived approximately 15 minutes after the inspector. Ten children's records, the provider's record, one household member's record, and the assistant/substitute provider's records were reviewed. The inspector arrived at the provider's home at 2:30 pm and departed at 4:45 pm. The children were observed during nap, during afternoon snack, during free play time inside, during outside time, and during departures. There was discussion with the provider about the following: an immunization record for a child (#10) who had documentation of the last immunization only.
Standard #: 22VAC40-111-60-B Description: Based on record review, the provider failed to ensure that three of 10 children's records reviewed contained complete documentation of required information. Evidence: 1. The last documentation of the parent reviewing the information in the child's record was 7/2/14 for child #1. There was no documentation of the review of the child's emergency contact information as required by 22 VAC 40-111-780 B. Children's records are required to be reviewed at least annually and documentation by the parent of any changes and to verify review of the record. 2. The last documentation of the parent reviewing the information in the child's record was 8/25/14 for child #3 There was no documentation of the review of the child's emergency contact information as required by 22 VAC 40-111-780 B. Children's records are required to be reviewed at least annually and documentation by the parent of any changes and to verify review of the record. 3. The record of child #10 was missing documentation of a first date of attendance and documentation of a second (required) emergency contact for the child (name, address and phone number to contact in case the parents can not be reached). According to the provider, the child has been in care since July 2, 2015. Plan of Correction: The provider will have the parents update and complete the children's records. In the future, the provider will ensure that when a child is enrolled the record is complete. The provider will review children's records monthly to catch any updates needing to be done.
