An unannounced monitoring inspection was conducted on October 9, 2015. There were four children present at arrival totaling 12 points on this date. The children were observed playing freely, listening to music and napping during the inspection. The menu indicated that today's lunch would consist of hot dogs, peas, bananas and milk. No medications are being administered in the family day home. Transportation is not provided by the family day home. Two children's record were reviewed during the inspection. The provider's record and two household member's record were reviewed.

Description: Based on observation and inspection of the family day home, the provider had not reviewed the emergency plan at least annually and update the plan as needed. Evidence: During the inspection conducted on October 9, 2015, the last review of the emergency plan for the family day home as documented on the plan was 7/1/2014.

Plan of Correction: The plan will be reviewed and the date will be updated.

Standard #: 22VAC40-111-170-A
Description: Based on review, the provider did not obtain from each adult household member prior to coming into contact with children a current Report of Tuberculosis Screening. Evidence: During the inspection conducted on October 9, 2015, there was no documentation of a TB screening in the record for Household Member #3.

Plan of Correction: We are in the process of getting her appointment.

Standard #: 22VAC40-111-320-G
Description: Based on review, the provider did not ensure that wood burning stoves, fireplaces and associated chimneys were inspected annually. Evidence: During the inspection conducted on October 9, 2015, the most recent chimney inspection for the family day home had expired and was dated 4/28/2014.

Plan of Correction: An inspection will be scheduled.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review, in three of three household records reviewed, the provider did not ensure that any person living in the family day home have required repeat background checks before three years since the date of the last check. Evidence: During the inspection conducted on October 9, 2015, there was no documentation of a criminal history check and a search of the central registry in the records for the Provider and Household Members #2 and #3.

Plan of Correction: Paperwork has been completed, required forms will be notarized and put in the mail.
