Comments:
Conducted an unannounced monitoring inspection. Observed 7 children = points with the Provider, Assistant #1, and recently hired Assistant #2. Ratios were in compliance and supervision was constant. Children were having diapers changed, going down for naps, and having quiet play time during the inspection. The daycare areas of the home were found to be clean, safeguarded appropriately, and sufficiently supplied with toys and equipment for the children. While many of the children eat baby food brought from home, lunch served included: macaroni and cheese, chicken nuggets, and milk. Required documents were posted. No medications reported to be on-site. Record keeping violations found. Inspection completed 11:40am-12:20pm.

Description: Based on records reviewed, it was determined that a recently hired caregiver did not have written reference checks on-file. Evidence: Assistant #2 - Date of hire was 9/14/15.

Plan of Correction: Reference received on 9/25/15

Standard #: 22VAC40-111-180-A
Description: Based on records reviewed, it was determined that an adult living in the home does not have a current TB screening form. Evidence: Adult #1 - Previous TB test expired in August 2015.

Plan of Correction: Tested on 10/7/15
