Technical Assistance:

The licensing inspector conducted an unannounced renewal inspection on from Friday, October 9, 2015 from 1:20pm to 2:20pm. Some children were finishing lunch while others were resting on cots. Lunch consisted of beef noodles, hot dogs, cabbage, bananas, pineapples, and milk. Today, there were six children, totaling 19 points, present with two caregivers. Interviews were held with the caregivers throughout the inspection. The provider does not administer medication. The provider does provide transportation. The provider does not not have pets. Three children?s records, two caregivers? records and one household record were reviewed during the inspection. The violations from the previous inspection in July of 2015 were checked for corrections. There were no repeat violations found during today?s inspection. Standard #: 22VAC40-111-100-A
Description: Based on a review of children's records on 10/09/2015, the provider did not ensure one of three records reviewed contained a physical within 30 days of attendance. EVIDENCE: The earliest documentation of a physical exam in the record of Child #1, who started at the family day home on 5/28/2015,is dated 8/25/2015. The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

Plan of Correction: Going forward, will request a physical at the same time as the immunization records, prior to the children starting the family day home.

Standard #: 22VAC40-111-520-A
Description: Based on observation and caregiver interview on 10/09/2015, the provider did not ensure that a crib was provided for infants under 12 months of age. EVIDENCE: Child #3 is two months old. There wasn't a crib available for the child to sleep in. According to Caregiver #1, Child #3 rests in a mesh side baby rocker that was provided by the child's parent. Cribs shall be provided for children from birth through 12 months of age.

Plan of Correction: Will purchase a crib today, 10/09/2015,in the meantime will have parent provide a crib.

Standard #: 22VAC40-111-530-G
Description: Based on observation on 10/09/2015, the provider did not ensure that children under the age of two did not use pillows. EVIDENCE: Child #2, who is 13 months, Child #4, who is 22 months, and Child #5 who is 15 months were observed resting with pillows on their cots. Children under two years of age shall not use pillows.

Plan of Correction: Will send pillows home and let parents know.
