Comments:
An unannounced monitoring inspection was made on 10/28/15 at 9:15 a. m. and concluded at 10:45 a. m. The inspector met with the provider and Family Day Home standards were reviewed. The following staff-to-child ratios and activities were observed today. There were 11 family day home children plus the provider's child present with points equaling 26 with two caregivers providing supervision. The provider was conducting circle time with the children, the inspector observed the children singing songs, going over the month, year, and days of the week. The children played a game "Red Light, Green Light" , working on the letter "E", and exercising to a video. Two children records were reviewed. Caregiver and household member records were reviewed. One observation conducted. Conversations were held with two children and two caregivers. No medications are given. The last emergency monthly evacuation drill was conducted on 10/15/15.

Description: Based on review of children records the provider did not have all required information on file. Evidence: On 10/28/15, there was no documentation available for proof of identity and age for child #1.

Plan of Correction: I will get a hard copy of this from the mother. This will be done tomorrow.
