Today an unannounced monitoring visit was conducted with the Provider-Ms. Munoz this visit took place in the afternoon between 12:15-2:45 there were 3 children present with the Provider for a total of 12 points within the supervision guidelines. The children were observed having snack, nap, lunch, listening to music, and free play. Lunch today consisted of spaghetti, fruit and milk. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider record, fire drill log, menu, and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-60-B Description: Based on observation of the children records, one record did not include documentation of the parent's work address. Evidence: Child #1's record did not not include father's work address. Plan of Correction: Provider will contact the parent today to get the information.
Standard #: 22VAC40-111-280 Description: Based on observation of the physical plant, cleaning agents were found in a storage closet outside of the bathroom unlocked and accessible to children. Evidence: A storage closet across from the bathroom, was found open which contained cleaning materials and hazardous substances. Plan of Correction: Provider immediately locked the door during this inspection.
Standard #: 22VAC40-111-450-D Description: Based on observation a door was found open for ventilation however the door did not have a screen. Evidence: Upon arrival, a patio/back door was found open to let air in however it did not have a screen. Plan of Correction: Provider stated that she was in the process of taking out a diaper when i arrived and forgot to close it.
Standard #: 22VAC40-111-540-A Description: Based on observation of the children during snack/lunch, two children were sitting in high chairs however one was not being used according to the manufacturer's instructions. Evidence: Upon arrival, one child was observed in a high chair without a safety strap. Plan of Correction: Provider immediately put the safety strap around the child's waist.
