Discussed with Provider that a substitute provider can only be used for 240 hours while she is out of the country.
Today an unannounced renewal inspection was conducted with the Provider-Ms. Kashfa. This visit took place in the morning between 9:00-11:30 there were 5 children present with the provider and 3 assistants for a total of 20 points within the supervision guidelines. The children were observed having morning free play, breakfast, snack, puzzles, blocks, nap and in transition for nap time. A complete inspection of the physical plant, children and provider records, fire drill log, medication and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-800-B Description: Based on a review of the provider emergency plan, the provider did not have documentation that she reviewed and updated the plan. Evidence: the emergency plan posted had not been updated since 2014. Plan of Correction: Provider stated that she will update the emergency plan today.
Standard #: 22VAC40-111-960-E Description: Based on observation, three bottles were found without the child's name and date. Evidence: Three infants were present however three bottles did not include the name and date. Plan of Correction: Provider stated that the child is new and the parents forgot to label the bottles. Labeled during this inspection.
Standard #: 22VAC40-111-60-A Description: Based on a review of the children records, three of the records were found not up-to-date. Evidence: Child #1-3's record, did not include documentation that the records were updated annually by the parent. Plan of Correction: Provider stated that she asked the parent to review the record however they did not sign off on the form. Provider will ask parents to sign the forms today.
Standard #: 22VAC40-111-60-B Description: Based on a review of the children records, one record did not include documentation of two or more designated emergency contact person name, address and phone number. Evidence: Child #5's record, only had one emergency contact person name, address and phone number listed. Plan of Correction: Provider stated that last Friday was suppose to be the child's last day and that the parent refuse to provide another emergency contact person.
Standard #: 22VAC40-111-250-C Description: Based on observation of the cribs, soft material was found drapped/tied across the top of the crib while a 6 month old infant was sleeping. Evidence: In the infant nap room, a 6 month old infant was observed sleeping in a crib with a soft material (crib bumper) drapped/tied across the top of the crib. Plan of Correction: Provider immediately removed it and stated that the child is starting to teeth and she placed it on there to prevent to child from biting on the top of the crib. Provider stated she will no longer use it.
Standard #: 22VAC40-111-410 Description: Based on observation of the physical plant, three portable electric fans were found on tables in the childcare area. Evidence: 3 fans were found out of reach of the children, however they were not mounted to prevent them from being able to fall over on the children. Plan of Correction: Provider removed the fans during this inspection and will make sure they are mounted.
