An unannounced monitoring inspection was conducted today. There are 7 children (18 points) with 2 adults. 1 adult and 3 children's files were reviewed. See violation notice for missing information. Please review all children's files for required documents. At the initial inspection, you were still within the 30 day requirement for physicals to be on file. Children were observed dancing, reading a book, singing and enjoying time to play freely. Emergency escape drills, bathroom supplies, fridge temperature and bottles are all in compliance. Standard #: 22VAC40-111-100-A Description: Based upon a review of files, one children's file does not contain a physical within 30 days of attendance. Evidence: Child 1 does not have a completed physical on file. (Date of enrollment/licensure date 8/24/15). The missing physical was noted on the Iast inspection as being within 30 days of necessary. Plan of Correction: I will ask the parent to provide a complete physical for the file.
Standard #: 22VAC40-111-90-C Description: Based upon a review of files, one child's file does not contain an acceptable immunization record. Evidence: Child 1's immunization record is not signed, stamped or electronically signed by the physician's office. Plan of Correction: I will ask the parent to provide a stamped or signed record.
