An unannounced monitoring inspection was conducted on November 24, 2015 from 4:24 p.m. to 5:12 p.m. Upon arrival, there were 2 infants, ages 3 months and 9 months, with a caregiver. A tour of the facility was conducted. Infants interacted with their caregiver and played with toys while listening to music. Observations included the provider's report to the parent who arrived to pick up a child. There was no medication being administered. An area of non-compliance was found in the administration section of standards. It is identified on the violation notice and was discussed with the provider during the exit interview.
Standard #: 22VAC40-111-60-B Description: Based on a review of 2 records, the facility did not ensure that each child's record had the name, address and telephone number of two designated persons to contact in case of an emergency if the parents cannot be reached. Evidence: The record of child #2 did not have the name, address and telephone number of a second emergency contact. The provider confirmed this. Plan of Correction: The provider is going to contact the parent this evening after daycare closing to get an emergency contact.
