Technical Assistance:
Consult and documentation provided regarding the Consumer Product Safety Commission (CPSC) Mandatory Standards for Cribs.

Comments:
An unannounced monitoring inspection was conducted beginning at 9:00 a.m. and ending at 11:30 a.m. There were five children in care ranging in age from nine months to four years of age for a total of sixteen points. Children were observed watching an age appropriate television program. Children were observed having free play with blocks and sensory boards and imagination play with a play kitchen. An infant was observed sleeping. Information regarding crib compliance must be forwarded to the licensing office within thirty days. If cribs do not meet the CPSC standards, cribs must be removed. Three children's records, two pet records, one provider record and two household member records were reviewed. Standard #: 22VAC40-111-100-A
Description: Based on review of documentation, the provider failed to obtain documentation of physicals within thirty days of attendance. Evidence: Child A, first day of attendance 9/16/15, Child B, first day of attendance 5/18/2015 and Child C, first day of attendance 6/9/15 did not have documentation of a physical in the file.

Plan of Correction: This information will be requested from the parents again. All parents were contacted during the inspection.

Standard #: 22VAC40-111-240-B
Description: Based on observation, the provider did not ensure cribs were in compliance with Consumer Product Safety Commission (CPSC) regulations from 12/28/2012. Evidence: A Cosco portable crib, manufacture date 3/15/2002, was being used for an infant in care. There was another crib in use that did not have any documentation.

Plan of Correction: The manufacturers will be contacted for the documentation. If the crib does not meet the CPSC regulations it will not be used.
