54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted from 10:30 am - 11:45 am. There were 8 children in care with the provider and one assistant ages, 2 months, 8 months, 12 months, 12 months, 12 months, 18 months, 2 years, and 3 years for a total of 27 points. The children were observed in free play. Lunch is provided from home. Eight children's records were reviewed. The provider, assistant, and one household member record were reviewed. The most recent fire drill was conducted 11/23/2015. The provider is current in CPR, First Aid and MAT (Medication Administration Training). The provider is not currently dispensing medication to children in the family day home. Standard #: 22VAC40-111-100-A Description: Based on review of eight children's records it was determined that not all children had documentation of a physical before 30 days of enrollment. Evidence: Child A with a start date of 9/17/2015 did not have documentation of a physical on file. Plan of Correction: The provider requested a copy of the physical for child A during the inspection.
Standard #: 22VAC40-191-40-D-1-c Description: Based on record review of one household member record it was determined that criminal history background checks were not repeated every three years. Evidence: The most recent criminal history check on file for household member A was dated 11/26/2012. Plan of Correction: The provider will submit the paperwork to repeat the criminal history check for household member A.
