54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted today, December 15, 2015 to determine the provider's compliance with licensing standards. In attendance were two children totaling five points being supervised by the provider. Children were interacting with the provider and using the restroom in preparation for lunch. Today's lunch consisted of chicken nuggets, french fries, applesauce, and milk. Children's books, toys, and equipment were inspected and found to be age appropriate and in operable condition. The children's restroom was inspected with the required supplies present. All rooms of the home used by children were inspected with no hazards found.
The family day home had the required postings displayed on the bulletin board upon entering the home. The first aid kit, fire extinguisher, and emergency supplies were inspected. Three children's and one provider record were inspected with the required information found. The following was also inspected: fire drill logs, shelter-in-place drills, emergency preparedness plan, parent agreements, and other pertinent information. See attached notice for violations found on this day.

Standard #: 22VAC40-111-690-D Description: Based on the inspection of the diapering table, the diapering mat was in unsatisfactory condition. Evidence: During the inspection of the diapering table, the diapering mat was found to have tears thus making the mat adsorbent to fluids. Plan of Correction: Per provider: I will replace the mat within the next week.
