- Technical Assistance:
Technical assistance today included attendance, menus, crafts and cribs. To find forms for your staff and on-line classes, go to dss.virginia.gov and go to Children, and Provider Resources and Training.
An unannounced monitoring inspection was conducted today from 1:30 - 3:15. There were seven children present plus two of the assistant's children for 33 points with the provider and two assistants. I observed children involved in quiet play, napping and eating snack. No children's medication was on-site. The outdoor areas were not inspected. Standard #: 22VAC40-111-940-A Description: During today's inspection, the provider was not able to demonstrate that the temperature in the refrigerator was maintained at 40 degrees or below as required. Evidence: The thermometer on the refrigerator door read 46 degrees, and the inside thermometer was not registering anything. Plan of Correction: I will turn down the temperature and buy another thermometer.
Standard #: 22VAC40-111-100-A Description: A review of records today found that there was no documentation of a physical exam for a child who has been in care over 30 days. Evidence: There was no physical for Child # 3 (who began care 8/15.) Plan of Correction: I will talk to his mother.
Standard #: 22VAC40-111-120-B Description: A review of staff/household records today found that there was some written information missing for one staff member. Evidence: Staff # 2 did not have two references documented. There was also no written information regarding educational background and emergency contacts. Plan of Correction: I will get these in her file.
Standard #: 22VAC40-111-60-B Description: A review of children's records today found that some information was missing. Evidence: There was no health insurance number listed for Child # 1. There were no immunization records available today for Child # 4 and Child # 5. Plan of Correction: I will talk to the parents.
Standard #: 22VAC40-111-70-A Description: Before the child's first day, parents of two children were not provided with information in writing regarding emergency procedures as required. Evidence: The parents of Child # 1 and Child # 3 signed the form but the information regarding the family day care home's procedures in an evacuation was not documented. Plan of Correction: I will fill it in and have them sign today.
Standard #: 22VAC40-111-80-B Description: A review of records today found that one child did not have verification of age and identification as required. Evidence: Child # 2 did not have written verification that the provider verified documentation of identification. Plan of Correction: I will ask his mother today.
Standard #: 22VAC40-111-180-A Description: A review of staff/household records found that one household member did not have documentation of a current TB screening. Evidence: Household member # 2's screening expired 1/15. Plan of Correction: He will get a TB screening.
Standard #: 22VAC40-111-200-C Description: A review of records today found that a staff member did not have written documentation of an orientation by the provider. Plan of Correction: I will go over the form with her.
Standard #: 22VAC40-111-240-B Description: During today's inspection, wires were observed to be accessible to children at child height. Evidence: The three infant swings had long wires that were accessible on the floor today. Plan of Correction: We will put them away.
Standard #: 22VAC40-111-510-C Description: During today's inspection, four occupied rest mats at naptime were observed not to be at least 12 inches apart from each other. Plan of Correction: Corrected during inspection.
Standard #: 22VAC40-191-40-D-1-c Description: A review of records today found that a staff member did not have results of a current criminal check. Evidence: A returned record check for Staff # 1 was sent back for more information in September, but there were no results from it. Plan of Correction: I sent it back last week.
