The Provider inquired about the renewal application and requested a copy.
An unannounced monitoring inspection was conducted in the Family Day Home during the morning on December 17, 2015. There were 4 children in care. The children's ages ranged from one to five years. Based on their ages, the total points were eight. Reportedly seven children are currently enrolled. The Provider and the Assistant were present during the inspection. The children were observed during free play with blocks. The LI also observed the children during the lunch meal. A sample of 4 children's records and two caregivers' records was reviewed. Documentation of the practiced emergency drills was reviewed. The children were not taken outdoors for play due to rainy weather. Reportedly, medications are not administered. The Provider gave the LI a list of the dates the FDH will be closed for the holidays and a date for a February closing. Findings of the inspection were reviewed with the Provider. Standard #: 22VAC40-111-120-D Description: Based on documentation reviewed, the Provider failed to ensure each caregiver's record contained background checks as required by 22 VAC 40-111-130. Evidence: The LI reviewed records of caregivers 1 and 2, the sworn statements were not updated every three years. Documentation in the records was dated 08/2012. Other required background reports were updated in August and September, 2015. Plan of Correction: The sworn statements will be updated by both caregivers. Current documentation will be retained in each record.
