Technical Assistance:
Discussed with the provider: The emergency medical authorization should authorize the provider to obtain emergency medical care. Note: First Aid and CPR will expire in May of this year.

Comments:
Unannounced renewal inspection was conducted beginning at 10:10 and ending at 11:30. There was one child in care this morning, age 3 1/2. The child was observed playing and interacting with the provider. The provider's training exceeds the 16 hours that are required. Emergency escape drills and shelter in place drills are up to date, Reviewed two children's files - see violation notice Reviewed 3 family files - see violation notice. Standard #: 22VAC40-111-60-B
Description: Based on a review of 2 children's files, it was determined that one file was not complete. Evidence: The file for Child Bdid not have Insurance policy number, physical examination, Completed Information for parents and providers name on the emergency medical authorization.

Plan of Correction: The provider will notify the parent and will get the needed information within 30 days.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on a review of the family files, it was determined that there was one family member who has a CPS check that has expired. Evidence: The CPS Check for Family member #3 has a Central Registry Check that expired on 12/14/15. CPS Checks are to be redone prior to their expiration date every three years.

Plan of Correction: The form will be sent today.
