Technical Assistance:
Discussed with provider; stuffed animals with sleeping children, background search requirements.

An unannounced renewal study was conducted today. There are 6 children present (17 points) with one adult. Children were resting quietly during today's inspection. One adult and 3 children's files were reviewed. See violation notice for missing information. Emergency evacuation drills, menu, diapering standards are all in compliance. Due to case load adjustments, your next inspection will be with a different inspector. Description: Based upon a review of files, one adult file is incomplete. Evidence: Adult A's file does not include documentation of 2 references.

Plan of Correction: I did conduct the references, they have been misfiled. I will locate the documentation.

Standard #: 22VAC40-111-60-B
Description: Based upon a review of files, children's files are incomplete. Evidence: 1 of 3 files reviewed was missing documentation of the health insurance ID/policy number. (Child 1)

Plan of Correction: Parent was contacted during the inspection and asked to provide the missing information.

Standard #: 22VAC40-111-70-A
Description: Based upon a review of files, children's files do not contain acknowledgement of policies/procedures. Evidence: 1 of 3 files reviewed was missing documentation of acknowledgment of Liability insurance declaration, and provisions of the emergency preparedness plan. (Child 3)

Plan of Correction: I will have the forms signed tonight.

Standard #: 22VAC40-111-170-A
Description: Based upon a review of adult files, one file is incomplete. Evidence: Adult A does not have documentation of a current TB test/screening on file. (Date of Employment: 10/2015)

Plan of Correction: The staff person has had the test, but not had the results read.

Standard #: 22VAC40-111-570-A
Description: Based upon direct observation, the provider exceeded the allowable points per adult number. Evidence: There are 17 points present based upon the children's ages, with one adult caregiver. 1- 14 month old (4 points), 1-17 month old (3 points), 2 - 23 month old (6 points), 1-2 year old (2 points), 1-3 year old (2 points).

Plan of Correction: I will not be over points. Another adult is usually present, and will be from now on.

Standard #: 22VAC40-191-40-B
Description: Based upon a review of files, one adult file has a disqualifying background search on file. Evidence: Adult A's completed background search includes a conviction for a crime that disqualifies them from employment.

Plan of Correction: The staff person will no longer be employed at the home.
