Your training year runs from 2/14 to 2/15.

An unannounced renewal inspection was conducted today with the provider, who was present with five napping infants/toddlers/preschoolers for a total of 14 points. For lunch, the provider reported that she fed the children peanut butter and jelly sandwiches, pineapple, carrots and milk. The provider has First Aid/CPR certification through American Safety Health Institute which will expire on 8/30/16. The family pet, Toby's vaccinations are dated 4/20/15 through 4/20/18. There have been no injuries/accidents, the provider has Medication Administration Training but currently have no children on medications. Provider has more than the 16 training hours required for the year. Arrival time: 12:15 pm Departure time: 2pm

Standard #: 22VAC40-111-180-A
Description: Based on a review of the provider's files and an interview with the provider, it was determined that a tuberculosis test was no renewed every two years. Evidence: The tuberculosis test for Person A is dated 5/18/13.

Plan of Correction: I will have Person A up date the tuberculosis test.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on a review of the provider's files and an interview with the provider, it was determined that one person did not have a central registry finding on file. Evidence: Person B turned 14 years of age in 2/2015 and to date does not have a central registry check on file

Plan of Correction: I thought Person B needed the sworn disclosure statement, I will make sure he gets the central registry check.
