The provider and assistant present with 8 children totalling 22 points. The children are napping, one of the children are sleeping upstairs in the living room . The children have lots of space for the children to play and toys for them to play with. The providers records were reviewed during the visit and the provider is up do date on her trainings and all other paperwork. The children had rice, veggies and cheese for lunch with fruit and milk. Description: The provider failed to record the shelter i.n place drills for the year. Evidence: Based on the review of the provider records and interview with the provider, the provider conducted the shelter in place drills but did not record them

Plan of Correction: I will make sure I record them.

Standard #: 22VAC40-111-170-A
Description: The provider failed to get a TB screening for the assistant. Evidence; Based on the review of records the provider has a new assistant who has been with her since 12/08/15. The caregiver needs a TB screening.

Plan of Correction: The assistant has an appointment for an xray, the doctor will not give her a screening.
