The play equipment is off limits until brought into full compliance. Make sure all play equipment is placed at least six feet from the perimeter of other play structures or obstacles and stationary play equipment is not installed over any hard surfaces. Please refer to all of the standards outlined under standard 500.
The provider self reported a situation involving care of children. Interviews were conducted as well as a review of one child's file and an inspection of the outside play area. Please submit your plan of correction for the violations by 1/25/16. Arrival time: 3:30pm Departure time: 5pm
Standard #: 22VAC40-111-500-C Description: Based on interviews and observation (on 1/20/16), it was determined that on 1/14/16, stationary outdoor play equipment was placed on a hard surface. Evidence: On 1/14/16 swings were placed over dirt ( grass was no longer under the swings due to wear/environmental conditions). Plan of Correction: We will no longer use the wings until the grass grows back or we are able to get mulch.
Standard #: 22VAC40-111-570-A Description: Based on observation on 1/20/16 at 3:30 pm and an interview with the provider, it was determined that one caregiver exceeded 16 points. Evidence: The provider was alone with 9 children for a total of 19 points for approximately 30 minutes. The assistant returned to the home after picking up a child from school. Plan of Correction: The assistant usually takes the older children with her when she has to pick up a child from school. Because it was very cold we decided to leave them at the house. We will not do that as of today. We will make sure the van is heated and children will go with the assistant so we will not be over points.
