Comments:
An unannounced renewal inspection was conducted Thursday, January 28, 2016, from approximately 12:00pm until 2:00pm. Present today were three children totaling seven points with the provider. The provider has a set curriculum each month. The theme for January is "Animal in the Snow". The children learned about polar bears. Letters covered this month were "Mm, Nn, and Oo". Today each children made an octopus. The children explained to the licensing inspector what to do and where to go in case of a hurricane, fire, sand storm, earthquake, volcano eruption, flood or tornado. Children have individual beds for naptime. The home provides linens. All required emergency items were present. The provider is currently certified in CPR and 1st Aid. No transportation Three children's records and two adult records were inspected. A full house walk-through was performed today.

Standard #: 22VAC40-111-830-C
Description: Based on review of documentation, the provider did not ensure monthly documentation of evacuation drills. Evidence: There was no documentation of practice evacuation drills for the months of November or December 2015.

Plan of Correction: Per Provider: Evacuation drills are practiced several times each month, however, I did not document these drills during the months of November or December 2015.

Standard #: 22VAC40-111-460-C
Description: Based on inspection of pet records, the provider did not ensure each pet had an updated rabies vaccination. Evidence: Out of four pet records inspected, Pet #1rabies vaccination expired 7/18/15.

Plan of Correction: Per Provider: I thought this had been done. I will take the dog to get an updated rabies vaccination.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on review of records, the provider did not ensure each person completed repeat background clearances as required every three years. Evidence: The provider's Central Registry Clearance expired 1/26/16. Household person #1 Criminal Record History expired 1/11/16.

Plan of Correction: Per Provider: Both individual's Criminal History Records have been mailed. The central registry clearances will be notarized today and mailed 1/29/2016. I will contact the licensing inspector upon receipt of these clearances.
